What's Included in Our Dental Plan:
For a low annual fee your coverage includes:
- Semi-Annual Dental Cleanings and Exams
- X-rays
- Fluoride Application
- Discounts on Dental Treatment(s)
Single pays a yearly enrollment fee of $375.00 (This covers 2 Exams, 2 Prophylaxis Cleanings, 1 set of x-rays; 4 bitewings 2 PA's, 1 Fluoride treatment per a year) In absence of gum disease (Periodontal Cleanings)
Any other treatment needed we'll give a 15% discount off our fees.
Married pays a yearly enrollment fee of $675.00 (This covers 2 Exams, 2 Prophylaxis Cleanings, 1 set of x-rays; 4 bitewings 2 PA's, 1 Fluoride treatment per a year) In absence of gum disease (Periodontal Cleanings)Any other treatment needed we'll give a 15% discount off our fees.
